Job overview
We are looking for a passionate and motivated Band 6 Occupational Therapist to join our Memory and Later Life Team. You will work alongside experienced members of the multidisciplinary team within the three distinct pathways (Early Memory, Complex Care and Care Home Education and Support Service). This post offers a variety of opportunities within the service, focusing on care homes to provide specialist Occupational Therapy assessments and interventions.
This role provides an opportunity to utilize core skills of Occupational Therapy in providing specialist assessment and interventions, while working alongside patients and their carers to promote engagement in meaningful occupation.
You will be part of a hardworking and caring team providing specialist input to people living in care homes and the community, with cognitive difficulties, mental health needs, alongside other physical health, neurological and complex conditions.
Main duties of the job
* To be responsible for the assessment and management of a caseload of service users with a variety of complex Occupational Therapy needs.
* To work primarily within the Care Home Education and Support Service (CHESS), but across all pathways depending on clinical need.
* In partnership with Service Users and Carers, develop collaborative intervention plans, ensuring regular reviews to monitor progress.
* To deliver evidence-based Occupational Therapy interventions.
* Coordinate treatment planning & interventions, reviews, and discharge planning.
* To work as part of the Multidisciplinary team alongside doctors, nurses, support workers, and care home staff.
* Provide clinical supervision and leadership to staff as delegated by Clinical Lead.
* To provide compassionate care based on empathy, kindness, respect, and dignity.
* To incorporate a client-centered philosophy into clinical practice.
Person specification
Education and Qualification
* Recognized Occupational therapy degree/qualification
* HCPC Registration
* Evidence of post-registration training in OT interventions relevant to the post.
* Member of Royal College of Occupational Therapy
Knowledge and Experience
* Awareness and understanding of professional accountability, HCPC and RCOT standards.
* Proven track record
