This is a fantastic opportunity to join the energetic, innovative Complex Discharge Team at Good Hope Hospital. You will provide clinical expertise to support safe, effective and timely discharge working directly with internal and external multidisciplinary teams as required.

Main Duties of the Job
1. Act as key-worker to co-ordinate individual complex discharges.
2. Act as Trust expert in discharge planning, assisting and supporting the MDT to drive and co-ordinate discharges, ensuring effective, timely intervention from all disciplines.
3. Assist wards and teams with understanding and using the 'Discharge to Assess' model.
4. Complete Health Needs Assessments to facilitate discharge.
5. Complete Fast Track Assessments for palliative patients.
6. Provide education in discharge planning to members of the MDT or other professional groups requiring this teaching.
7. Support transfers to NHS funded community venues/services.
8. Monitor performance associated with discharge, identifying gaps in service provision that may have a detrimental effect on length of stay, delayed transfers of care or patient experience.
9. Work with multi-agency representatives to reduce length of stay and delayed transfers of care, to achieve performance targets related to discharge.
Essential Skills
* Evidence of knowledge of the Hospital Discharge and Community policy.
* Evidence of knowledge of Discharge to Assess and the hospital discharge pathways.
* Ability to demonstrate the importance of, and commitment to integrated working.
Qualifications
* Registered Nurse.
* Degree or equivalent.
Experience
* Proven post-registration experience at ward level.
* Success in working collaboratively across professions and services.
* Working in a hospital environment.
