We have an exciting opportunity for an experienced and enthusiastic ANP to join our multi-disciplinary primary care team to improve the health and wellbeing of our patients and to support the development of our Nursing Associate in her journey to becoming a registered nurse.
Although working independently, you will work alongside our GPs, Pharmacists and Nursing Team. The focus for this role is minor illness along with providing support for woman's health and Long term conditions. Experience in Asthma, COPD and Spirometry would be advantageous.
This is a permanent position up to 27 hours. In addition you will be required to take part in the Extended Access rota on Saturday mornings and annual vaccaintion clinics. Applicants must have a minimum of 12 months prescribing experience. You will demonstrate safe, clinical decision- making and expert care for our patients both in the practice and in the patients home, if required.
Main duties of the job
Acting within your your professional boundaries provide a high standard of patient care, from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
Diagnose and manage minor illness, long term conditions, including drug and non-drug based treatment into a management plan; referring onwards to other members of primary, secondary and other external health care providers as appropriate.
Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence based practice, national and practice protocols and within scope of qualified practice.
This role will also be expected to cover home visits for housebound patients within your area of clinical expertise, supporting QOF data and for delvering NHS vacciantion programmes.

We are based 5 minutes walk for the town centre, with onsite parking, and are an active member of the WGC PCN with our neighbouring practices and part of the local Federation, Ephedra.
There is an experienced, friendly team of administrative staff including the Practice Manager, Deputy Practice Manager, Reception Manager & Team, Senior Administrator and Secretaries.
Our list size is growing and currently we have around 12 500 patients.
Job responsibilities
Work as part of a multi-disciplinary team to deliver safe and high quality care.
Carries out clinical nursing practice at an advanced level using clinical skills, critical judgements and knowledge to deliver appropriate and safe care.
Co-ordinate the planning and delivery of care ensuring patients and carers/relatives, where necessary, are involved.
Prescribe and review medication that is appropriate to patient needs, in line with latest national and local protocols.
Request diagnostic investigations and interpret within agree protocols e.g blood tests.
Assess, diagnose and treat patients in practice premises or their own home, referring to other agency or healthcare provider as appropriate.
Support with home visits for patients with minor illness, vaccination programmes and long-term-conditions with clinical area of expertise.
Assist and support development of other members of the nursing team.
