Working in partnership with the NHS continuing healthcare teams, our enhanced level of specialist clinical support enables individual clients to remain at home even when the level of health care required is highly complex. Our services are provided through local and specialty focused multi-disciplinary teams.
As an Adult Nurse Case Manager (Tracheostomy Experienced), you would be responsible for providing face to face support to our client and their families whilst creating personalised care plans for each client. You would work closely with a core team of carers training and supporting them with clinical skills and activities.

Key Requirements
22. · Registered Nurse with a minimum of 12 months’ work experience in the UK and an active NMC PIN
23. · Tracheostomy and ventilator experience desirable
24. · ITU/HDU experience would be desirable
25. · Care of the spinal patient knowledge desirable
26. · Enteral feeding experience essential
27. · Excellent written & verbal communication skills
28. · Ability to work flexibly and respond quickly to changing priorities whilst meeting deadlines
29. · Ability to build strong relationships with clients, their families, and carers
30. · Supportive management style that can bring out the best in others
31. · Confident, passionate and possess a “can-do” attitude
32. · Ability to manage your own caseload of community packages which includes working in the community and at Wimbledon head office
