An exciting opportunity has arisen for a Health Care Support Worker to join the @Home Service in a permanent position.
The post holder will work as a member of the @Home Service under the direction of the Team Leader.
The post holder will contribute to the implementation of nursing care as assessed and prescribed by the Registered Nurse (RN), undertake a range of delegated duties to meet the needs of specified adults in a variety of settings including patients' homes, clinic settings, residential and nursing homes. This includes working in a multi-disciplinary and multi-agency arena and in partnership with the primary health care team.
The range of responsibilities will be in support of and supervised by a Registered Nurse or Therapist, and will involve rotation as appropriate to the needs of the service. The post holder will acknowledge their limitations and actively seek advice as necessary.
Main duties of the job
The post holder will work within the @Home Service, which provides a comprehensive multi-disciplinary assessment service for adults presenting with complex needs in the community who are likely to be admitted to hospital in the absence of a timely health intervention.
The post holder will work closely with multidisciplinary team colleagues, contributing to the implementation of nursing care as assessed and prescribed by the Registered Nurse, undertake a range of delegated duties to meet the needs of specified adults in a variety of settings including patients' homes, clinic settings, residential and nursing homes.
We are looking for a skilled, caring professional with a commitment to excellence in nursing practice who will support the clinical leads with the monitoring of the @Home service performance and practice developments.

Job responsibilities
The post holder will be required to undertake a range of clinical and clerical activities as delegated by the Team Leader/ANPs/RNs. These activities will be carried out in a variety of settings including patients' homes, clinic settings, residential and nursing homes.
The post holder will also contribute to the maintenance of medical and surgical stock levels adhering to the agreed minimum and maximum stock levels required for the @Home Service.
The post holder will hold Level 3 NVQ/QSF/Apprenticeship 3 in a Health Care associated Certificate or possess skills, knowledge and experience to equivalent level. The post holder must be prepared to commit to begin the Level 3 qualification within three months of commencing in the role.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary: £24,433 to £26,060 a year pro rata
