A fantastic and rare opportunity has arisen for a part time community Assistant Practitioner to join the Coventry and Rugby Community Learning Disability Team.
This is a split post between nursing and occupational therapy. We welcome applicants with a passion for working with adults with a learning disability and a “can do” attitude, who reflect the values (compassion, respect, integrity, collaboration, excellence) of CWPT. This role provides care and support to those in their homes and services within these catchment areas.
We adopt a holistic approach and focus on preventing hospital admissions and placement breakdowns,emphasis on improving client’s quality of life.
Qualifications
Candidates require a Foundation degree in Health and Social Care (Assistant Practitioner) qualification or NVQ (Care) level 3 Four GCSE’s grade c or above (Maths and English desirable) or functional skills maths and English (level 2).
Experience
Experience of working in a Learning Disability practice setting
· Due to the nature of community working it is essential to hold a full driving licence.
· Experience of working to support people with a learning disability in distressed states including either significant mental health needs, behaviours of concern or offending behaviour.
· Experience of working in a multi-disciplinary team in the community.
· Experience of working with Nurses / Occupational Therapists.
· Experience of lone working within the community.
Working as part of a multidisciplinary team, assistant practitioners use enhanced skills in a specialised area to provide direct high quality and effective therapeutic care and support to clients including those with more complex needs. Within the scope of the role, the post holder will deliver individual or team tasks where the understanding of role boundaries and the importance of supervision and carrying out appropriately delegated tasks and plans is essential.
You will carry a varied caseload of clients and will work in and be supported by the experienced multi-disciplinary.

Please note that the ability to travel across the area is essential for this role, and we are looking for candidates who have worked alongside therapists and nurses or have experience in similar clinical settings before. • Working under the supervision of a qualified practitioner to modify, develop and implement programmes or interventions.
• To complete relevant assessments and care plans under the supervision of a qualified practitioner.
• To liaise appropriately with carers/families/clients/other professionals. This may include representing others at reviews and Multi-disciplinary Team meetings.
• To contribute to the development of risk management plans. Report to the qualified practitioner any incidents of risk to the service user or risk to others as a result of the behaviour or actions of the service user.
• To carry out, under supervision or direction, activities which optimise service user’s levels of functioning and the development of meaningful activities for individuals.
• To undertake administration and clerical duties as required.
• Take active ownership of own training and development needs, to include participation in the trusts performance management / supervision framework and work toward meeting identified goals and objectives as agreed in a personal development plan / appraisal
