Community Nurse Team Leader - Chipping Norton
We are recruiting for a Community Nurse Team Leader to join our team in Chipping Norton where you will have accountability and responsibility for the district nursing team.
You'll be key in providing strong clinical leadership, ensuring the delivery of expert evidence-based nursing care to patients whose needs are best met in the community, their own home or other care setting.

A minimum of 30hrs per week may be considered. Cross locality working and flexibility is required for this role.
Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.
Main duties of the job
* Provide strong clinical leadership and management of the team
* Accountable for the management, development, and supervision of identified staff within the community team
* Overall accountability and responsibility for the named district nursing team
* Clinical holistic assessment and treatment of complex patients
* Work in partnership to reduce hospital admission and facilitate discharge
* Promote optimal levels of independence and wellbeing for patients.

Qualifications
* Degree level educated
* Registered General Nurse Adult
* Evidence of level 7 academic study
* Frailty Module
Experience
* Formal management and/or leadership experience within healthcare
* Strong leadership skills. Ability to plan and organise own and others work.
* Previous District Nursing experience
Personal Qualities
* Ability to develop and sustain relationships with partner organisations and the public.
* Well-developed communication and interpersonal skills
