Main Duties:
* The post holder will be responsible to the Chief Pharmacist.
* Duties to be carried out in accordance with medicines and general law, professional ethical guidelines, Department of Health guidelines and Trust general and pharmacy policies.
* The post holder will be expected to work on his or her own initiative within these procedures under the direction of the Chief Pharmacist and Clinical Pharmacy Managers.
* The post holder will provide specialist clinical input to a specified area, developing the medicine education facilities for service users to help facilitate concordance, reduce relapse and foster a positive attitude to drug therapy.
* This will include clinically screening medicine charts promptly and resolving any issues at the time.
* Produce a medication history and medicines reconciliation for selected patients, including previous drugs taken, known adverse drug reactions, compliance problems, and identification of individuals at risk of drug-related relapse.
* Review individual medication psychotropic and general medicines and adjust in collaboration with senior medical staff.
* The post holder will review all medication use regularly, actively participate in multidisciplinary team meetings, and ensure optimum administration times.
* Ensure electronic and paper patient notes with regard to medication are accurate and up-to-date.
* Identify and fulfil specialist medicine support and education needs of existing Trust and other care staff.
* Act as a specialist medication referral point for Trust and other care staff to provide individual patient counselling and education.
* Liaise with, and provide advice to, GPs regarding medication optimisation.
* Advise Community Mental Health Teams and GPs regarding initiation of treatments in the community and necessary support for clients with medication-related problems.
* Support and assist in running the Trust medicine education (MedEd) programme.
* This will require effective communication skills with patients who may have barriers to understanding medication education and advice.
* Discharge medication counselling, using both written and verbal information.
* Provide pharmaceutical advice and support for relatives and carers, including relative support group development.
* Develop appropriate liaison with medical, nursing, pharmaceutical and other staff in the provision and development of Clinical Pharmacy Services.
Requirements of the Pharmacist:
* GPHC Registration
* Previous experience within a UK based NHS Pharmacy setting
* NVQ or equivalent – Pharmacist qualification
