Job summary We are looking to recruit a Care-Coordinator (PCN), based at and working directly with our team at Granville House Medical Centre. Care-Coordinators play an important role within our PCN/Practice to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services and ensure their needs are met. Bringing together all the information about a person's identified care and support needs and exploring options to meet these within a single personalised care and support plan, you will review patients' needs and help them access the services and support they required to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches and other professionals where appropriate. You will also be involved in specific recall systems to streamline the management of patients and ensure regular monitoring and reviews. Main duties of the job The role is for someone that is enthusiastic, hardworking, organised and enjoys working in a fast paced, supportive environment. Working closely with our GP's and practice team, you will manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed. With excellent communication skills and a compassionate and friendly nature, you will carry out telephone and face to face assessments with specific cohorts of patients, identifying any unmet needs and where appropriate referring them to the most suitable service. For more complex patients, you will participate in and MDT meeting to fully assess the patient with external healthcare services. You will support our clinicians and management team to provide excellent care to our patients and fulfil administrative functions that support the running of the practice.
