The Ophthalmology Team are responsible for provision and development of a modern Ophthalmology Service to the local population. You will be required to actively participate in the delivery of Ophthalmology services in collaboration with other health care professionals. You will need to have experience in nurse led clinics including IVT injections, triage, emergency support and other ophthalmic clinics. You will required to assist and run these clinics within Sherwood Forest hospital Trust at Kingsmill Hospital, Newark hospital and Ashfield sites. You will have support from the Ophthalmic team, the matron and specialist general managers. In return we would require you to be flexible, approachable and have good communication skills. You must be a good role model and should be able to follow and promote our CARE values and strategic objectives to help usto deliver a nurse led service for the clinical management of patients with Ophthalmic changes.
* The overriding purpose is to support the provision of the highest qualitypatient care through personal actions and continuous improvement.
* Whilst on duty provide strong clinical leadership, energising, inspiring and empowering others in pursuit of a shared vision to ensure the effective provision of patient care related to Ophthalmology.
* Ensure care delivery is in line with the appropriate regulatory body code practice (e.g. NMC).
* Ensure a quality-orientated approach to care, which recognises the needs and views of patients/clients and their carers (where applicable), placing the patient at the centre of all activities.
* Undertakes nurse led intravitreal clinics, emergency support clinics, triage and post op/macular clinics which establishes and maintains own workload in collaboration with Ophthalmology multidisciplinary team and demonstrates an ability to plan and work in accordance with the needs of the service.
* Undertakes triage and emergency clinics using SOP to evaluate and prioritise patients.
* Ensure specialist knowledge is such that any abnormalities can be identified and discussed with the medical team.

* Responsible for providing specialist evidence-based advice to all disciplines of staff, patients and carers relating to Ophthalmic conditions Provides specialist advice relating to individual patients and groups of patients in relation to Ophthalmic conditions
* Provides an efficient service and utilises full potential of their nurse led clinics.
* Contributes to the development of plans of care for patients who need input from the Ophthalmology team.
* Participates in the development of policies and guidelines relating to Ophthalmology, which may have impact in a variety of areas throughout the Trust.
* Uses audit and observation to ensure that other staff follow relevant policies.
* Has responsibility with other members of the team for ensuring that training is provided on Ophthalmic issues to all staff from a variety of professions.
* Participates in the evaluation of new equipment and services in relation to Ophthalmic conditions.
* Run Nurse Led Clinics to provide highly skilled care on behalf of the Ophthalmology Consultants for patients with Ophthalmic conditions g and provide specialist nursing advice for patients.
* Ensure a quality patient centred approach to care, which recognises the needs and views of patients and carers.
* Responsible for recording patient information. Both written and using computer software as appropriate.
* Only reveals sensitive information to relevant individuals from this data storage.
* Regularly participates in audit, research and surveillance. Contributes to initiation of these programmes related to Ophthalmology using a recognised audit methodology.
* Responsible for maintaining, developing and completing own personal portfolio and submitting evidence for revalidation as required.
* Responsible for maintaining mandatory training compliance.
* Responsible for reporting adverse incidents or errors where staff or patients may be at risk.
* Participates in and proposes modernisation, improvements and service development within the team.
* To undertake any other duties which may be reasonably regarded as within the nature of the duties, responsibilities and grade of the post as defined, subject to the proviso that appropriate training is given.
* Provide education and support to the patients, carers and staff.
