Warrington and Halton Core 24 is a specialist assessment of service users' mental health for those who present to the Emergency Department in times of crisis or are referred from acute general hospital wards. We focus on the formulation of treatment and risk management plans based on the assessment of needs with appropriate triage and diversion from the service.
The service operates on a shift system of long and short days (currently under review), inclusive of nights, to provide 24/7 service throughout the year. We have recently undergone Organizational Change to enhance flexibility across the Urgent Care footprint, providing support to additional Core 24 services including Royal Liverpool, Warrington, Whiston, and Southport Core 24 services where required.
We are looking for an experienced practitioner with extensive experience working in mental health services, including organic services.
Responsibilities include:
1. Providing high-quality, evidence-based care within the Division for service users and their carers in inpatient and community services.
2. Working in a socially inclusive way and in line with the recovery model.
3. Functioning as named nurse/care coordinator to service users, undertaking assessment, planning, evaluation, and review of care within a recovery-focused model.
4. Participating as an active member of the multi-disciplinary team.
5. Providing advice, expertise, and leadership within the team.
6. Supervising care for service users presenting with complex and high-risk needs.
7. Working autonomously, reaching clinically appropriate decisions based on information received and utilizing the knowledge and experience of the multi-disciplinary team.
8. Being qualified to act as a mentor for students.
