Job summary We are looking for a part-time Advanced Nurse Practitioner to act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate. The ANP will provide a wide range of health care including treating acute and minor illnesses, chronic diseases, triage and contributing to QOF requirements. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF. Process and interpret pathology & other test results as required. Support the team in dealing with clinical emergencies. Extended & supplementary prescriber. Contribute to practice targets (QOF etc.), complying with local & regional guidance. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.). Delegate clinical responsibilities appropriately (ensuring safe practice & the task is within the scope of practice of the individual).
