Job overview
The Navigator Nurse will be part of the Single Point of Access. The post holder will ensure that the needs of the patient/client referred for assessment are provided with the highest possible standards of practice.
The role of the Navigator Nurse is to bring together different professionals to implement a timely and person-centred assessment to determine appropriate care pathways for patients to avoid unnecessary hospital admissions. This involves partnership working across agencies at the interfaces of primary and secondary health and social care.
To ensure the outcome of the nursing element of the assessment is multi-disciplinary in approach and to arrange and coordinate community services, Intermediate Care Services, and Care Packages as required.
Main duties of the job
* Provide Leadership on community services to the staff working in the ED and the Hospital Integrated Discharge Team (HID) to ensure effective multi-disciplinary working.
* Lead on the fostering of excellent relationships, clear protocols, procedures, and documentation between the Navigator Nurse and staff in ED and the Urgent Care.
* Develop and maintain links with Community Matrons, Long Term Conditions Teams, Specialist Teams, and General Practitioners, developing clear protocols, procedures, and documentation – ensuring the patient is kept at the centre of the care plan.
* Build effective working partnerships with ED staff to prevent unnecessary admissions, build links with Urgent Care staff, ensuring effective use of community services.
* Be professionally and legally responsible and accountable for all professional activities.
* Work closely with other health professionals and social services and provide nursing advice in meeting the needs of the patients.
* Recommend appropriate interventions to promote independence and safety, utilising effective clinical reasoning skills and evidence-based practice.
* Maintain effective multidisciplinary communication with other members of the team and the wider clinical network to ensure that client’s needs are met.
* Participate in MDT meetings and family conferences as required.
* Ensure management of clinical risk with clients under their care.
