Job Overview West Essex Community Health Services Respiratory Team have an exciting opportunity for a part time Senior Support Worker. This role will cover all of West Essex Localities. The role will require a good knowledge of Community Specialist Services working with the Senior Nursing Team to care for patients with Long Term Respiratory Conditions. The applicant will require excellent communication skills and general administration skills to ensure a high standard of record keeping via our EPR system Regrettably, we cannot offer sponsorship for all of our job roles. Main duties of the job The post holder will work as part of the Community Respiratory Specialist Team which aims to provide ongoing care and monitoring for patients with Long Term Respiratory Conditions and prevent unnecessary admission to hospital. The role will include monitoring and supporting patients who attend community clinics and pulmonary rehabilitation programmes, visiting patients in their own homes, and providing telephone support and advice. The post holder will require good clinical skills appropriate for the role and will be an excellent communicator. Some administrative skills will be required, which will involve maintaining computer records and ensuring a high standard of record keeping.
