Main duties of the job o Perform comprehensive assessment of patient care needs. o Plan, implement and evaluate care according to changing health care needs. o Provide supervision of junior staff and support to other team members. o Collect and evaluate report information and maintain accurate patient records, to enable effective care delivery. o Involve the multidisciplinary team and where possible patients, carers/relatives in the planning and delivery of care. o Establish and maintain effective communication with patients and the multidisciplinary care team. o Recognise and respond appropriately to urgent and emergency situations. o Seek/act on advice from a senior team member or relevant other, as appropriate o Develop own skills and knowledge and assist in the development of others. o Act as a mentor/coach and assessor to pre and post registration learners. o Lead responsibly for a specific function or area of knowledge/skills within the team. About us Alder Hey Children's NHS Foundation Trust is a provider of specialist health care to over 275,000 children and young people each year.
