Main duties of the job An integral part of the East (Flintshire and Wrexham) Integrated Health Community (IHC), your work will ensure primary care and wider community services are developed and deployed effectively, serving the local population in a whole-system approach, so people receive the right care, at the right time, from the right service. You will work alongside cluster and professional collaborative leads, and IHC members, to develop the roles and functions of clusters and professional collaboratives. A great facilitator and communicator, you will be responsible for providing support to clusters and other networks, ensuring they can respond to population needs through effective and robust planning and service delivery, whilst leading development and innovation at pace. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
