A Neighbourhood Team Community OT role is fast-paced and responsive to both urgent and planned referrals working closely with other nursing and physiotherapy roles. Making the patient's home safer and guiding the promotion of independence is where the OT really adds value to the patient experience. Assessments are holistic and focused on maximising independence at every stage. Patients are at the heart of everything we do.
Good opportunities exist for further training and development within the role. You will receive support and development through in-service training, regular appraisals, a personal development plan, and clinical supervision.

Main duties of the job
A day shift is 7.5hrs between the hours of 8:00am to 6:30pm Monday to Sunday including bank holidays. If you do work weekends, it will attract an enhancement payment. Flexible working arrangements considered.
Your typical day will include coordinating your caseload, triaging referrals, allocating workload, and prioritising urgent response assessments alongside colleagues. You will perform holistic, goal-focused, patient-centered assessments, and develop thorough treatment programmes to meet the goals. You will manage a varied caseload, spanning all Occupational Therapy specialities.
The role will involve you:
* Undertaking occupational therapy assessments for a designated caseload, addressing occupational performance and skill deficits, enabling the client in areas of self-maintenance, productivity, and leisure.
* Assessing clients in relation to their equipment and specialist equipment needs.
* For minor adaptations e.g. rails, half steps.
* Maintain contemporaneous clinical notes to professional minimum standards of the College of Occupational Therapy and in accordance with Trust guidelines.

Job responsibilities
The post holder will:
* Work as an autonomous practitioner within an integrated team providing timely and expert professional assessment and treatment interventions to patients.
* Provide case management for a designated caseload of patients who have immediate and/or complex care needs.
* Triage patients to ensure prioritisation of clinical need. Develop and co-ordinate individualised rehabilitation plans in close liaison with the team, the patient, and their carers as well as all other services.
* Be responsible for the timely assessment and treatment of patients who have urgent and/or complex Occupational Therapy needs.
* Co-ordinate appropriate interdisciplinary, patient-centred care that promotes independence for patients on their caseload.

* Able to meet the travel requirements of the role
Knowledge
* A range of evidence-based Occupational Therapy clinical skills and knowledge appropriate to specialism
* An understanding of the approaches and models of practice relevant to practice.
* A good level of knowledge in a wide range of physical and mental health conditions
* Good level of knowledge relating to disability, equipment, and adaptations
* A working knowledge of Occupational Therapy Outcome Measures and Models of Practice
Experience
* A range of post-graduate experience including work in a variety of physical and/or mental health settings/specialities
* Experience of working in a multidisciplinary/interdisciplinary team
* Experience of caseload management and prioritisation
* Experience of working in an integrated team
Skills & Abilities
* Ability to work proactively with other agencies to prevent hospital admission and facilitate early discharge
* Ability to identify and plan individual care/rehabilitation needs in partnership with patients/ their carers
* Supervisory/Appraisal skills
* Knowledge of appropriate policy and guidelines
* Confidence in passing on skills/knowledge and completing teaching sessions/presentations
Qualifications
* Degree/diploma in Occupational Therapy
* Current registration with HCPC
* Evidence of relevant post-graduate courses/Continual Professional Development maintained in portfolio
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer
