The West and South Norfolk Care Group are excited to advertise for a Band 6 Senior Physical Health Nurse to work with our Crisis Resolution & Home Treatment (CRHT) team and our team on Samphire Ward at Chatterton House in Kings Lynn to improve the physical health care of mental health service users in accordance with directions and standards in line with Trust Policy, Strategy and National Guidance.
The post holder will act as an expert resource within NSFT by working in collaboration with members of the physical and mental health team, providing advice and guidance to support the effective and safe delivery of the physical health agenda.
Main duties of the job
The Physical Health Nurse will:
1. Provide a wide range of physical health monitoring and brief intervention treatment services to service users.
2. Train and supervise other Trust staff: nurses, clinical support workers and medical staff on clinical and physiological observations, new equipment, monitoring tests and physical care interventions.
3. Recognise changes in routine physiological observations and escalate these to the relevant referral route to enable efficient management of illness and disease.
4. Work with the Lead Physical Health Nurse to provide ongoing disease management through use of co-produced care plans.
5. Provide advice on smoking cessation, weight management and sexual health initiatives to service users.
6. Assist with maintaining a comprehensive network of link persons from within and external to the Trust and ensure that best practice is communicated and cascaded to the team, supporting Trust and NHS(i) initiatives that enhance the wellbeing of service users.
7. Relay guidance to Trust staff on the safe use of medical devices and monitoring equipment (including assisting with selection and teaching use).
8. Support the team with audit processes relating to specific monitoring of the service provision by CRHT and Samphire Ward.

Person Specification
Qualifications
* Registered Nurse
* Leadership and management qualification
* Post registration qualification in health promotion including smoking cessation, sexual health and well person screening
Knowledge
* Knowledge of public health and NHS improvement initiatives, well person screening, physiological monitoring tests and selection of interventions or referral options for wound care, respiratory illness such as COPD, diabetes, sexual health and family planning.
* Recognised accreditation in Quality Improvement
Experience
* Experience of developing, delivering and evaluating training particularly around behaviour/lifestyle change
* Experience in creating, implementing and reviewing audit data.
* Experience of Mental Health
