This role is part of a multi-disciplinary team based with Forensic Outreach and Liaison Service.
This is an integrated role working closely with the local authority and other stakeholders such as probation, Ministry of Justice, and the criminal justice system.
The post holder will be responsible for creating an environment that is safe, supportive, and easily accessible with a no wrong door approach.
You will provide time-limited 1:1 support to clients, giving information and/or support to allow them to access a wide range of agencies and services that can provide help and advice.

3. Providing increased support to our clients when transitioning back into the community.
Job responsibilities
Key Responsibilities include:
1. Supporting the service with developing an assertive outreach approach when engaging with clients in the community.

4. Undertaking low-level mental state examinations in the community/hospital.
5. Becoming involved in a sub-group that will focus on moving clients through different levels of hospital security under the supervision of qualified staff.
6. Imparting highly complex and sensitive information to people using the service and their families/carers regarding assessment, care planning, treatment, and review.
7. Contributing to the team's engagement with a wide range of stakeholders including staff, people using the service, their carers, GPs, probation, MOJ, criminal justice, and other statutory and voluntary sector agencies.
8. Liaising with a wide range of other professionals and agencies to ensure the provision of consistent, well-coordinated care.
9. Contributing to the safe and effective day-to-day operations of the service including the handling of clinical inquiries.
10. Supporting risk assessments and risk management plans.
11. Ensuring that the needs and views of carers, friends, and family are considered.
12. Supporting arrangements for carer assessments and referrals to local authorities as required.
13. Ensuring that interventions are planned in an integrated and coordinated manner.
14. Supporting access to appropriate information and guidance around medication.
15. Supporting assessment, planning, intervention, and evaluation of care packages and Direct Payments.
16. Ensuring that appropriate action is taken to safeguard adults and children.
17. Contributing to the achievement of the key performance indicators for the Trust.
Person Specification
Qualifications
* NVQ level 3 in Health Care or equivalent experience.
* Counselling and/or some psychological training and skills would be an advantage.
Experience
* Minimum of three years experience working within a mental health setting or with people who experience mental illness.
* Ability to demonstrate experience of working towards an environment that promotes user involvement and recovery.
* Experience of working in Multi-Disciplinary Teams and/or with other agencies.
* Ability to identify changes to clinical risk and implement actions to ensure a person's safety.
* Experience in delivering plans of care that support physical, psychological, and social needs.
* Ability to demonstrate an understanding of the concepts of clinical risk assessment.
