Job summary An exciting opportunity to join a forward-thinking and specialist multi-disciplinary team to work with patients with chronic pain within the Frimley ICS. We are looking for someone who has an interest in chronic pain and is passionate about empowering patients to develop the skills and strategies to self-manage their condition. Ideally you will have experience of working alongside patients with chronic pain or long-term conditions and using movement based therapy and breathwork as tools for improving quality of life. Our multi-disciplinary team consists of Pain Consultants, Clinical Nurse Specialist, Clinical Health Psychologists and Advanced Practice Physiotherapists that work closely together to deliver comprehensive assessments and formulate treatment plans. The biopsychosocial model underpins our practice, and we all strive to work in a psychologically informed way. This is supported by ACT, trauma informed care, CBT and mindfulness.CPD is supported throughout the team and this is achieved through in-service training and external courses. As part of the team you will have access to a CPD funding and links with a practice development for Allied Health Professionals. Main duties of the job To take a leading role in the advanced assessment, clinical diagnosis and treatment of patients with Chronic Pain. To use highly specialist, physiotherapy knowledge and skills to manage a complex caseload as an autonomous practitioner. To ensure a thorough in-service training programme is provided to all members of the team to maintain clinical competency. To act as a specialist resource for the multi-disciplinary team and Physiotherapists within FHFT. To work with the MDT to develop, monitor and evaluate the Chronic Pain service, enhancing pathways in line with national guidelines To lead in appropriate research and audit in the specific clinical area. To support the Advance Practice Physiotherapist and deputise for them as appropriate.
