An exciting opportunity has arisen for an enthusiastic registered nurse to join the Adult Bladder and Bowel Specialist Service. The team are part of Nottingham CityCare Partnerships Adult Services, based at Sherwood Rise Health Centre to provide specialist continence related care to housebound clients within Nottingham City.
Incontinence is a pervasive health issue and its impact on the quality of life for clients, their families and carers are widely acknowledged. However, the fact that bladder and bowel symptoms can be an indicator of more serious underlying health conditions, is less recognised meaning every person who experiences bladder and bowel symptoms should have their condition holistically assessed to understand the aetiology.
You must have a driving licence and access to your own transport.
The core hours of the service are 08.30 - 16.30hrs Monday to Friday.

Job responsibilities
Job Purpose
* Complete holistic continence assessments and re-assessments, translate outcomes into individualised management and treatment plans (including onward referral if necessary), implement care required and evaluate the effectiveness for adult clients within a community setting.
* Provide continence advice to clients, carers and those who work with clients with continence problems, through clinical practice, teaching, research and development, as part of an integrated team.
* Maintain contemporaneous records as an autonomous practitioner.
KEY RESPONSIBILITIES
CLINICAL
* Be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care. This may include onward referrals depending on symptoms and clinical history.
* Ensure a high standard of clinical care for the clients under your continence management, and support staff to do likewise.
* Determine by assessment and liaison with other relevant agencies the advice, education and treatment requirements of each client and or carer.
* Undertake the comprehensive assessment of clients using investigative and analytical skills through standardised and non-standardised assessments.
* Formulate individualised management and treatment plans, using clinical reasoning and a wide range of treatment skills and options to devise a programme of care and carry out treatment interventions.
* Accept clinical responsibility for a designated caseload of clients and organise this effectively regarding clinical priorities and use of time.
* Provide advice, teaching and training to other members of the health care team regarding continence promotion and management.
* Gain valid informed consent and identify through assessment clients who lack capacity and work within a legal framework with clients who lack the capacity to consent to treatment.
* Work as a lone practitioner with supervision from band 6 / 7 continence advisor. The Team Leader will also provide clinical supervision.
* Provide spontaneous and planned advice, teaching and instruction to clients, carers and other professionals to promote understanding of continence treatment and management, and to ensure a consistent approach to client care.
MANAGERIAL AND LEADERSHIP
* Effectively undertake line management responsibilities and provide support to Assistant Practitioners as appropriate.
* Act as a professional role model aligned with Trust values and behaviours and implement a compassionate and coaching style of leadership.
EDUCATION AND DEVELOPMENT
* Ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames (as detailed in the competency guidelines).
* Be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Also, to keep an active up to date CPD portfolio.
PROFESSIONAL
* Participate in the development of good practice in continence care within Nottingham CityCare Partnership under the guidance of the Adult Bladder and Bowel Specialist Nurses and the Clinical Service Manager.
* Support and encourage reflective practice in continence care, promoting a culture of continuous improvement.
* Demonstrate a commitment to seeking out and delivering evidence-based practice.
* Be responsible for own Continuing Professional Development under the supervision of the Adult Bladder and Bowel Specialist Nurses keeping abreast of new trends and developments, incorporating them into your work as necessary. Attend relevant conferences and internal/external training courses.
* Maintain a high level of nursing standards and work to the Nursing and Midwifery Code of Professional Conduct.
* Actively participate in the services training strategy, including in-service training, Trust and multi-agency training. Participate by attendance and presentation at staff meetings, tutorials and training sessions.
* Support the development, implementation and evaluation of established continence link nurse / worker schemes.
* Demonstrate a sound understanding of Clinical Governance and Risk Management and apply to the work situation.
* Assist with development and participation in audit projects, evidence-based practice projects and outcome measures, either individually or with other senior therapists. Make recommendations for change.
* Participate in the staff Appraisal Scheme and Personal Development plans.
CLINICAL QUALITY AND GOVERNANCE

ORGANISATIONAL
* Support the Adult Bladder and Bowel Specialist Nurses in implementing objectives to assist in the ongoing development of the Service. Participate in service evaluation and advise the Adult Bladder and Bowel Specialist Nurses on the service needs of the client population.
* Liaise effectively with other professionals and agencies to enhance continence care within Nottingham CityCare Partnership.
* Manage the day-to-day delivery of own caseload within the Adult Bladder and Bowel Service. Also organise own diary and time management. Arrange assessments and appointments for own caseload.
* Always ensure the health & safety of staff and clients in accordance with Health & Safety legislation and Trust policy and procedures. Adhere to all other Trust Policies and Procedures.
* Attend team meetings and meetings with other agencies as required.
* Use information technology and participate in identified data collection and statistics and maintain clear, concise and timely records in line with the service policy.
* Undertake any other duties which may be required, to meet the needs of the service.
Person Specification
Qualifications
* Clear evidence of continued professional development.
* Mentorship Programme, e.g. supporting practice learning or equivalent.
* ENB 978 or B72/73 Promotion of Continence certification or similar equivalent qualification, or clearly demonstrated experiential learning.
* ENB 998 or City and Guilds 730 or equivalent qualification / experience.
* Non-medical prescribing (V300).
Experience
* A keen interest in promotion of continence and acting as an effective patient advocate.
* A clear understanding of health inequalities and evidenced experience of working with diverse or disadvantaged groups.
* Evidence of participating in health promotion activities with clear examples.
* A clear understanding of safeguarding principles and evidence of effectively working with vulnerable adults.
* Practical experience of clinical supervision.
* Catheter competencies for male, female, and suprapubic catheters.
* Evidence of participating in service development and quality improvement, giving examples to support.
* Experience of direct classroom teaching.
Special Criteria
* Ability to be flexible over hours worked within contracted hours to meet the needs of the service.
* Full driving licence and the ability to travel between locations (own transport).
