Cedar Oak Healthcare Services is looking for a dedicated Care Coordinator to join our team and assist us with progression.
As a Care Coordinator, you will offer support to care workers and managers. You should be able to communicate with families of service users as well as other professionals such as doctors, commissioners, social workers, occupational therapists, nurses and others. You should be able to provide advice and guidance for carers on the field. You are expected to keep your training up to date and attend routine meetings with the local authorities and other healthcare providers when necessary.

Managing and supporting workers so they can perform their roles safely and to the best of their ability;

Acting as a positive role model to colleagues;
Developing care plans that outline what care and support individuals need;
Planning and reviewing service ROTA;
Leading team meetings and doing inductions, supervisions and appraisals;
Conducting supervisions, spot checks and appraisals;
Conducting meetings and disciplinary procedures;
Submitting KPI and other necessary reports
Working in partnership with other professionals to ensure individuals get the support they need.
