The role of a care coordinator is both challenging and fulfilling, serving as the initial point of contact for the elderly facing mental health challenges and significantly influencing the care we deliver in our community. We are seeking a candidate with the following qualities: - Exceptional communication and collaboration skills - A dedication to person-centered care - Robust initiative and self-motivation - Outstanding teamwork capabilities - Proficiency in advanced nursing interventions and community assessment techniques - Readiness to participate in relevant training programs - A fervent commitment to quality enhancement Main duties of the job The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to co-ordinate the care of a complex caseload of service users supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate. Working under the supervision of the Deputy Team Manager/Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
