We are looking for a Practice Nurse to join our diverse team. The ideal candidate will be dynamic and highly motivated with general practice experience. You will be NMC registered with strong interpersonal and communication skills and it is imperative you are a team player.
Main duties of the job
* Cervical Cytology
* Wound care and dressings
* Chronic disease management (COPD & Asthma)
* Routine and child immunisations and vaccines
* Knowledge and understanding of QOF
* Knowledge and experience of working within the framework of Locally Commissioned Services and Direct Enhanced Services

We are an EMIS practice but full training will be given if you are not familiar with EMIS.
Job Responsibilities
* Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients' health
* Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
* Provide vaccination and travel medicine services
* Provide wound care (ulcer /Doppler etc.) to patients
* Provide cervical smear tests
* Request pathology services as necessary
* Process pathology results as required
* Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
* Maintain accurate clinical records in conjunction with extant legislation
* Ensure read codes are used effectively
* Maintain chronic disease registers
* Develop, implement, and embed well woman clinics
* Develop, implement, and embed well man clinics
* Implement vaccination programmes for adults and children
* Prioritise health issues and intervene appropriately
* Recognise, assess, and refer patients presenting with mental health needs
* Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
* Support the clinical team with all safeguarding matters, in accordance with local and national policies
* Deliver opportunistic health promotion where appropriate
* Maintain baby immunisation results logs
* Maintain smears results logs
* Manage emergency drug kit

Qualifications
* NMC registered

* A willingness to work under supervision/training and assessment in the general practice setting
* Positive attitude, ability to gain trust and support with patients, carers, and work colleagues.
* Ability to do phlebotomy work
Experience
* Thorough and up-to-date knowledge of nursing theory and best practice.
* Understanding of equality & diversity and how to apply it.
* Understanding of NMC Code of Practice and requirements of it for own practice & behaviour.
* Knowledge of primary health care and General Practice.
* Knowledge of the role of General Practice Nurse.
* To have gained effective experience and skills from primary care or community student placements.
* Trained on EMIS clinical system

£26 to £30 an hour depending on the experience
