Job summary
We are currently recruiting a Clinical Lead to be responsible and accountable for ensuring the delivery of safe, effective and efficient care to service users. To be responsible and accountable for the delivery of effective and high-quality evidence-based care and support packages. Oversee and manage all nursing and care teams, including rota management. To provide clinical advice and leadership to designated staff within the Organisation and contribute to appraisal and supervision.
The successful candidate will need to demonstrate clinical excellence in relation to clinical reasoning and decision making when managing complex cases. Support the Clinical Development Lead and Clinical Support Manager in their roles with team development and assist the registered nurses, Senior Practice Nurses and Team Leaders in their daily roles. Provide direct clinical interventions at an Advanced Practitioner level to an agreed number of service users. Demonstrate commitment to the provision of compassionate care that is based on empathy, kindness, respect and dignity.

Provideexpert clinical support and guidance to the clinical and care team in thedelivery of services to residents at Chaseley.
Chairmultidisciplinary clinical meetings including, complex clinical reviews andRisk Strategy Meetings. Liaise with other professionals, agencies andstakeholders across the care pathway ensuring that service user transitionsadhere to standard work and are safe and timely.
Actas a clinical role model and motivate staff to deliver high standards of careand support within the clinical service.
Demonstrateexcellent communication skills and effectively provide and receive sensitiveinformation, using developed interpersonal qualities, in highly emotiveenvironments.
Provideverbal & written Reports and advice as requested.
Demonstrateexcellent professional working relationships within the clinical team, withTrust colleagues and external partners.
Ensurethat where there is a barrier to communication or understanding, that the teamare able to access appropriate services to deliver the care pathways, such asInterpreters, Advocates, Clinical advice from other professionals.
Supportthe Trust, Team & Clinical Managers in the management and resolution of teamconflict acting as part of the overall management team. Use de-escalation,negotiation skills when dealing with complex, hostile and emotive situations.Utilise communications in accordance with Caldicott Principles, DataProtection, Freedom of Information Act and Trust Policies and procedures.
Completesix-monthly reviews with each service user, their relatives and or otherprofessionals and provide a written clinical update.
Ensureservice user care plans and risk assessments are person centred and kept up todate in line with any physical, psychological and emotional changes.
Contributeto the clinical audit as outlined by the Deputy Manager.
Ensureeffective clinical quality monitoring programmes are in place.
Inconjunction with the Practice Educator, ensure relevant formal and informalquality improvement sessions are planned and delivered.
Contributeto the Clinical Governance quality review meetings.

Providea visible clinical leadership role to the designated staff working within thelocality, supporting and engaging staff in providing a high-quality clinicalservice to Users and Carers.
Promote,Develop, and Implement clinical standards and good practices both in caredelivery and appropriate record keeping.
Toliaise with Deputy Manager and Clinical Support Manager to ensure thecompletion, submission, liaison and review of all CHC, FNC and DoLS relatedplanning, monitoring, discussions and documentation.
SupportRegistered Manager and Deputy Manager in reporting, notification,investigation, and documentation of all safeguarding related issues within theOrganisation. This includes ensuring that learning points are always sharedwith staff.
Assess,Interpret and Analyse a range of complex clinical information and environments,delivering a clear rational for care delivery, clinical outcomes and clinicalmanagement strategies.
Demonstratea high level of reflective practice and competence in clinical reasoning anddecision making.
Demonstrateexcellent analytical and judgmental skills in relation to Initial and ongoingAssessments and subsequent Treatment Packages and assist practitioners inproblem solving and providing quality care.
Developchampions within the team for Mouth care, wound care, PEG care, Nutrition,catheter care, falls and continence.

Experience
Essential
1. Hold a relevant Professional Qualification & ensure this is kept up to date
2. Evidence of post qualified clinical experience and training and practice at an Advanced Practitioner level.
3. Have a proven track record of working collaboratively with adults experiencing substantial and complex problems.
4. Demonstrate specialist clinical knowledge underpinned by theory and experience, demonstrating excellent clinical reasoning and decision making skills.
5. Evidence of clinical supervision skills, and a working knowledge of a variety of clinical supervision models including live supervision and group supervision. Evidence of effective positive Risk Taking and Risk Management & Safety Planning.
