What You Will Do: Assessment and Intervention: Conduct comprehensive assessments in line with the Social Services and Wellbeing (Wales) Act, ensuring a person-centred approach. Identify the needs of children and young people with disabilities and develop tailored care and support plans. Service Delivery: Provide support and advocacy to children, young people, and their families, empowering them to navigate barriers and access appropriate services. Implement and monitor care plans, ensuring they remain responsive to the evolving needs of service users. Work collaboratively with citizens, carers, and families to enhance resilience and promote independence. Legislative Compliance: Fulfill local authority duties to disabled individuals and their carers, ensuring compliance with relevant legislation and guidance. Ensure safeguarding practices are adhered to, identifying and responding to risks appropriately. Collaboration and Multi-Agency Work: Engage with multidisciplinary teams, including health, education, and voluntary sector professionals, to deliver integrated and holistic care. Participate in case reviews, team meetings, and professional development activities. Requirements: Essential: Degree in Social Work (or equivalent qualification). Registration with Social Care Wales. Experience working with children and young people with disabilities. Strong understanding of the Social Services and Wellbeing (Wales) Act and other relevant legislation. Excellent assessment, planning, and communication skills. Ability to manage a complex caseload independently. Desirable: Experience in multi-agency collaboration.
