This role provides a unique opportunity for an Occupational Therapist (OT) to work as part of an integrated, community-based Urgent Community Response (UCR) service in North West Surrey. The post holder will work as part of a multi-disciplinary team to deliver holistic assessments, interventions, and ongoing support to adults with a variety of complex needs, ensuring safe, high-quality care is provided within the community setting. The OT will provide a rapid assessment and short-term interventions to prevent hospital admissions and enable patients to live independently for as long as possible. The post holder will be responsible for delivering specialist Occupational Therapy interventions within the UCR team, including conducting assessments, providing therapy, and supporting the rehabilitation of individuals.
