Hywel Dda University Health Board are currently looking to recruit a part-time Community Psychiatric Nurse to join the Older Adult Pembrokeshire Community Mental Health Team based at St Brynachs, Haverfordwest.
The post is permanent and will be for 30 hours per week.

The post holder will be responsible for the assessment of care needs, the development of programs of care, and the implementation and evaluation of these programs of care for a defined clinical caseload, including the role of a Care Coordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.
The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies, maintaining good communication across all boundaries.
