Job summary We are looking for a doctor to join our exciting new programme in Furness - The CHIEF Programme - as Clinical Lead. We are developing a new multidisciplinary team to provide dedicated support for those patients who are at high-risk of admission to hospital and those who are recurrently being admitted. The programme aims to be C o-ordinated, H olistic, I ntensive, E arly and F ocussed, bringing together colleagues from medicine, nursing, pharmacy, social care, voluntary sector, therapy and community teams. Working in an MDT environment you will provide clinical input and leadership to the team as you collectively define a management plan which addresses the needs of the whole patient across physical, mental and social care needs. Clinical Leadership This new programme will require strong and innovative clinical leadership. The successful applicant will need to develop strong relationships with the teams involved in the MDT and wider system partners. This is an opportunity to shape an exciting new model of care in Furness. The Clinical Lead will also have responsibility for line managing clinicians working within the programme, ensuring high quality clinical care and participating in trust quality and safety work. Doctors within the CHIEF programme will provide leadership of MDT meetings, clinical reviews (either remotely or face to face via home visits or clinic appointments) and support to the wider team with senior decision making.
