Job summary Poole Town Surgery is looking for a friendly and professional Practice Nurse with an NMC registration to join our team. We are looking for someone to work between 11 hrs per week. Main duties of the job Responsible for implementing processes for the effective management of patients with long-term conditions using evidence-based practice. The post holder will ensure that nursing services such as dressings, annual reviews, cervical smears, vaccinations, and immunisations are effectively delivered to the entitled patient population. You will ideally have experience in the management of diabetes. Responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure. There may be, on occasion, a requirement to carry out other tasks.
