Salary: £29,970 to £38,308 (dependent on experience)

What You’ll Do
* Deliver high-quality care to individuals in their own homes.
* Work as part of an award-winning, multi-disciplinary team.
* Provide compassionate, patient-centred nursing care while promoting independence and recovery.
You Will
* Conduct first visits, address immediate care needs and report to the Team Leader for further discussion or assessment.
* Provide care according to the available evidence of best practice and the support of others in the development of evidence-based practice.
* Actively involved in promoting self-care for patients and building Health Promotion into care.
* Develop and participate in planned teaching programmes for patients.
This role also requires you to have a full UK driving licence and either: access to a vehicle for work purposes, or the ability to travel to meet service needs (not all locations may be accessible by public transport).
