Job summary The Older Adults Community Mental Health Team in Trafford is an integrated health and social care service and strives to provide quality recovery-based support to people with organic and functional mental health problems. Under the line management and clinical supervision from within the team, the post holder will maintain and co-ordinate timely and appropriate interventions for a caseload of service users living in the community within the framework of CPA, in line with relevant clinical pathways as outlined within the Older Adults CMHT operational procedure.
