Job summary Applications are invited for the post of full time Care Navigator for our busy GP Practice located in the heart of Sittingbourne town centre. Working hours are 37.5 hours per week, Monday to Friday, on a split shift rota system. Some evenings and early mornings are required. This role is for 12 months fixed term contract. (Maternity Cover) Main duties of the job The successful applicant must be flexible, be able to work under pressure and enjoy working within a team. A good standard of education and previous experience of working in a GP surgery or in a customer facing role are essential. Duties can include but are not limited to, greeting and directing patients, effective use of the appointment system, booking appointments, processing of information and assisting patients as required. Answering the telephone to patients and outside agencies. To act as the central point of contact for patients, the distribution of information, messages and enquiries for the clinical team, liaising with multidisciplinary team members and external agencies such as secondary care and community services.
