About your new role
You will provide person-centered, bio-psycho-social treatments within a family-inclusive context via Telehealth, digital, and face-to-face sessions at our Horsham Hub or through outreach across the Wimmera Grampians region.
You will deliver care coordination, therapeutic interventions, and AOD support for consumers needing higher levels of care under the stepped care model.
You will offer expert psycho-social assessments and evidence-based treatments to support recovery from chronic mental illness and dual diagnosis, with flexibility for consumers to adjust their level of support as needed.
About the service
Step Thru Care is an integrated Mental Health and Alcohol and Other Drugs (AOD) support service available to support people with low-level needs to mild and severe mental health issues and or AOD support needs.
Uniting works in partnership with Grampians Community Health to deliver Step Thru Care services in the Wimmera Grampians region.

It’s a full-time role until 30 June 2026. We can also consider a 9-day fortnight or four days per week if that works better for you.

What you will bring
You are a skilled mental health clinician capable of delivering treatment, care coordination, and AOD support to consumers needing more intensive care within a stepped care model.
You will also bring with you a:
* A tertiary qualification in social work, psychology, counselling, mental health nursing, or occupational therapy, and full registration with the relevant professional body.
* A current driver’s licence valid in Victoria.
* Sound knowledge and understanding of the underlying principles of Mental Health and AOD recovery support gained through experience, education or training.
* Knowledge of the statutory requirements for Community Mental Health and AOD services.
* Experience delivering clinical counselling services that improve the outcomes for consumers with a clinically diagnosed mental disorder through evidence-based treatment.
* Demonstrated ability to provide early intervention strategies and case management.
