Job summary The Rural West Primary Care Network (PCN) is seeking to recruit a second Health and Wellbeing Coach, to work within our PCN's multidisciplinary healthcare team and enhance our friendly, busy network of practices. The main purpose of this role is to help make a difference to patients' lifestyles, decision making and future choices in respect of physical health and wellbeing. Working alongside our existing clinical teams, our current Health & Wellbeing Coach and Social Prescribers, this role will enhance individual patient case management, follow up and ongoing support. The Health & Wellbeing Coach will support the national 'Better Health' campaign to encourage weight loss; they will also support patients where lack of fitness/poor nutrition is contributing to or impacting on a mental or physical health condition, causing physical symptoms. In addition, the postholder will lead on the development of a range of initiatives that will benefit the health and wellbeing of our communities such as Park Runs and Park Walks. In line with the 'WorkWell' initiative, they will support people who are disabled and those with health conditions to rejoin the workforce. Main duties of the job The Health & Wellbeing Coach will work closely within a multidisciplinary team with patients of low to medium complexity that will usually have one or more long-term conditions. The postholder will have a proactive approach to preventing illness in patients for whom obesity is a major risk factor for ill health in the future. They will work with a diverse range of people from different cultural and social backgrounds, so the ability to work confidently and effectively in a varied and sometimes challenging environment is essential. Applicants will need an appropriate professional sports qualification and a valid supervision contract with a suitable professional Body. They will also have experience in health promotion and training in sports and exercise, which will include motivational coaching, behavioural change and goal setting. Also required are excellent interpersonal and communication skills and the postholder will be organised, energetic, patient and empathetic. The postholder will also preferably have experience of working in a primary care setting, ideally in health, social care or other support roles including direct contact with people, families or carers. About us The Rural West Primary Care Network (PCN) is friendly, busy network of practices, comprising two GP Partnerships: Tadley Medical Partnership and Watership Down Health with surgeries in Kingsclere, Overton, Oakley and Tadley, Hampshire. The post is part time (26 hours per week over 5 days) and requires travel within the Network to the surgeries of both Partnerships and communities.
