Job Description:
As a Care Coordinator, you will be responsible for overseeing the care of patients by coordinating health services, managing schedules, and communicating with various stakeholders such as doctors, carers, and family members. You will ensure that our clients receive the necessary resources and support to meet their medical, social, and emotional needs.
Key Responsibilities:
* Serve as the primary point of contact for patients and their families.
* Develop, monitor, and adjust care plans in collaboration with healthcare providers.
* Coordinate appointments, treatments, and services between healthcare professionals.
* Ensure patients have access to necessary resources such as home care, therapy, or transportation.
* Maintain accurate patient records, ensuring compliance with healthcare regulations.
* Monitor patient progress and report any changes in condition or needs.
* Provide emotional support and guidance to patients and their families during care.
* Collaborate with multidisciplinary teams to ensure a holistic approach to patient care.
* Advocate for patients' rights and ensure they receive the highest standard of care.
Requirements:
* Level 3 in healthcare, social work, or a related field (preferred).
* Must hold a full driver Licence and access to own car.
* Previous experience in care coordination, case management, or a similar role.
* Strong understanding of healthcare systems and community resources.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Proficient in using healthcare management software and basic computer applications.
* Exceptional organizational skills with attention to detail.
