About the role:
1. Leading a team of two prescribing pharmacists, 3pharmacy technicians, 1 trainee pharmacy technician / medicines managementcoordinator, 1 care coordinator
2. Main priority is currently prescribing safety,CQC reports, auditing, with some telephone / face to face clinics for patientreviews
3. Supported by a PCN Manager and ClinicalDirector, Prescribing Lead GP Partner, and wider clinical workforce

Main duties of the job
The Key Responsibilities for the role will be:
8. Oversee MOF allocating the work streams to the team and implementing a project plan
9. CQC audits - to delegate these through the team and ensure completed every month with clear rationale for why patients may remain on list
10. Oversee the safe review of MHRA and CAS alerts and ensure dissemination and learning.
11. Support for Dispensary Manager to review SOPs, attending dispensary meetings
12. Formulary support for Prescribing Lead Partner GP
13. Conduct appraisals for all staff within the pharmacy team
14. Oversee the safe prescribing of high risk monitoring in line with LMSG and ensure regular audit and robust safety processes
15. EPS - supporting GPs to sign EPS as needed
16. Supporting with medication queries and reviews as well as managing the flow of work through the system
17. Lipid management
18. QOF - to support in areas of review needed to achieve QOF
19. PCN DES Extended hours - to support and review the workforce delivering this model
20. Green agenda - reviewing inhalers and recycling possibilities and other aspects of the green agenda for general practice as related to meds
21. Scripts Workflow Support the scripts desk in any acute queries on the day and assist GPs in signing of EPS scripts
22. Honorary contracts with the Market Harborough Pharmacy and to support in services Hypertension, Pharmacy 1st, NMS, contraceptive reviews

Key Objectives:
23. Providepro-active leadership on medicines and prescribing systems to the practice clinicalpharmacy team, patients, and their carers
24. Developand deliver patient facing services and clinics which improve patient outcomes,reflect the needs of the practice population and shift workload from other disciplines
25. Deliverhigh quality medication reviews and develop bespoke medicines treatment plans forpatients (particularly the elderly, complex patients, and those at high risk ofadmission)
26. Improvethe quality and effectiveness of prescribing through clinical audit and educationto improve performance against NICE standards, prescribing guidance, and other clinicalstandards
27. Developyourself and the role through participation in clinical supervision, training, andservice redesign activities
28. Delivertraining, mentoring and guidance to other clinicians and staff on medicines issues
Patient Facing Clinical Services & Care
29. Workin a patient facing role to clinically assess and treat patients using theirexpert knowledge of medicines for specific disease areas
30. Takeresponsibility for the care management of patients with chronic diseases andundertake clinical medication reviews to proactively manage people with complexpolypharmacy, especially the elderly, people in care homes, those with multiplelong-term conditions (in particular COPD and asthma) and people with learningdisabilities or autism (through STOMP - Stopping Over Medication Programme)
31. Prescribeacute and repeat medication for patient need within areas of competency
Medicines Optimisation Systems
32. Provideleadership on person-centred medicines optimisation. Through structuredmedication reviews, support patients to take their medications to get the bestfrom them, reduce waste and promote self-care
33. Overseesystem for reconciliation of medicines from outpatient and discharge letters andsupport safe transfer of care through liaison with hospital and community colleagues
34. Takea central role in the clinical aspects of shared care protocols, clinicalresearch with medicines, liaison with specialist pharmacists (including mentalhealth and reduction of inappropriate antipsychotic use in people with learningdifficulties) and anticoagulation
Safety
35. Reportmedicines related incidents, contribute to investigations and root causeanalyses
36. Participatein serious incident investigations and multidisciplinary case reviews
37. ReportAdverse Drug Reactions and promote reporting by MDT
38. Actionand respond to Drug Safety Alerts and other safety cascades.
Training, Education & Information
39. Developand deliver education sessions and training to clinical and non-clinical staff onmedicines issues
40. Providead-hoc medicines advice to doctors and other clinicians
41. Assessemergent clinical evidence and develop strategies for implementation in practice
42. Horizonscan for new drugs and provide information to the MDT on their introduction
43. Provideadvice on cost effective prescribing and supply issues

Qualifications
Essential
59. An undergraduate Masters Degree or equivalent in Pharmacy.
60. Membership as practising pharmacist with The General Pharmaceutical Council.
61. Commitment towards continuing professional development in accordance with requirements of The General Pharmaceutical Council.
62. Pharmacist independent prescriber registered with The General Pharmaceutical Council.
Desirable
63. Post-graduate qualification to Diploma or Masters level in a recognised area of pharmacy practice or advanced clinical practice.
