As a Dementia Nurse Practitioner, you will:
Support, deliver, and coordinate person-centred dementia care.
Implement therapeutic interventions to improve clinical outcomes.
Work within a multidisciplinary team of nursing, social care, psychology, and wellbeing practitioners.
Conduct holistic nursing assessments.
Provide general nursing and bio-psychosocial interventions.
Share knowledge and deliver best practices.
Use critical reflective practice.
Operate as an autonomous practitioner.

The ideal Dementia Nurse Practitioner will have:
An active NMC Pin.
Minimum of 2 years' practice experience in a dementia care community.
Evidence of delivering person-centred care for people with complex dementia needs.
Experience in collaborative and multidisciplinary team working.
Strong communication skills, flexibility, and creativity.
Commitment to quality care and professional development.
