Groves Health is looking for an experienced and motivated Practice Nurse to deliver high quality care to patients at our Hinchley Wood Surgery.
Ideally we are looking for a Nurse to work 4 days a week. Working hours will be by arrangement. You may be required to work at one of our other sites on occasions, which are in: Richmond, Wimbledon and New Malden.
Main duties of the job
Conduct thorough assessments of patients' health conditions.
Formulate and implement care plans.
Provide treatment as necessary.
Perform treatment room duties and CDM clinics.
Collaborate with other healthcare professionals to ensure optimal patient care.
Maintain accurate and up-to-date patient records.
Offer health education and counselling to patients and their families.
Stay current with the latest medical advancements and best practices.
Job responsibilities
Primary Responsibilities:
1. Identify, manage, and support patients at risk of developing long-term conditions.
2. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
3. Provide vaccination and travel medicine services.
4. Provide wound care (ulcer/ Doppler etc.) to patients.
5. Request pathology services as necessary.
6. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/ specialist care as required.
7. Maintain accurate clinical records in conjunction with extant legislation.
8. Ensure snomed codes are used effectively.
9. Maintain chronic disease registers.
10. Maintain vaccination programmes for adults and children.
11. Provide cervical cytology.
12. Prioritise health issues and intervene appropriately.
13. Recognise, assess, and refer patients presenting with mental health needs.
14. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice).
15. Support the clinical team with all safeguarding matters, in accordance with local and national policies.
16. Deliver opportunistic health promotion where appropriate.
17. Monitor and maintain baby immunisation targets.
18. Monitor cervical smear results.
19. Manage emergency drug kit.
20. Attend clinical and practice meetings and clinical supervision.
Secondary Responsibilities:
1. Develop, implement, and embed health promotion and well-being programmes.
2. Implement and evaluate individual treatment plans for chronic disease patients.
3. Assist GPs with procedures when required.
4. Chaperone patients where necessary.
5. Support the team in dealing with clinical emergencies.
6. Liaise with external services/ agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
7. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
8. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
9. Act as the audit lead, effectively utilising the audit cycle.
10. Monitor and ensure the safe storage, rotation, and disposal of medicines.
11. Support junior members of the nursing team, providing guidance when necessary.
12. Participate in local initiatives to enhance service delivery and patient care.
13. Support and participate in shared learning within the practice.
14. Continually review clinical practices, responding to national policies and initiatives where appropriate.
15. Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA).
Person Specification
Experience
1. 2 years' relevant experience working in a primary care environment in the last 2 years.
2. Experience of chronic disease management.
Qualifications
1. Nursing degree or equivalent experience.
2. Registered Nurse (NMC).
3. Evidence of immunisation training (childhood, adult and travel health).
4. Experienced in complex wound dressings/leg ulcer management.
5. Evidence of cervical cytology training.
6. Standards for Students Supervision & Assessment (SSSA).
7. Diabetes Diploma.
8. Family Planning qualification.

1. Immunisations (flu, routine, childhood, B12, travel).
2. Wound Care/ Treatment of small injuries/ Removal of sutures & staples.
3. Women's health (Cervical cytology, contraception, etc.).
