We are recruiting a Digital and Transformation Lead to work with our member practices in the North West Doncaster Primary Care Network (PCN). This role will be based within the North West Doncaster PCN practices. Main duties of the job Networks of general practices, currently constituted as PCNs under the national Network Directed Enhanced Service commissioned by NHS England, are a key building block of the health and social care system. The North West PCN mission is to strengthen general practice and work with communities and health and care system partners to personalise care, improve population health and reduce health inequalities. This role, employed by PCDand embedded in Doncaster North West PCN, is responsible for the development and implementation of the PCN Strategic Plan which sets out the ways in which the PCN will carry out its mission and in particular supporting the adoption of new technology and other initiatives to improve the care offer and enabling PCN staff to work more effectively to support the sustainability of general practice services. The post holder will manage and co-ordinate the activities of the PCN so that its strategic objectives are met and will provide leadership to the network alongside, and under the supervision of, the Clinical Director.
