What you will be doing?
The Health and Wellbeing Practitioner delivers client-centered treatment depending on the requirements of the individual client. You will be required to manage a caseload, provide appropriate harm reduction advice and guidance, assess, care plan, and deliver structured 1:1 and group-work sessions for both Substance Misuse and Low Level Mental Health service users. Transitional support from prison to the community via integrated working with other Health and Wellbeing agencies both within the prison and in the community is an essential element of the work to prepare service users for release.
1. Develop and contribute to the Substance Misuse and general Health and Wellbeing care needs of service users, supporting them in the development of release plans which are appropriate to their needs.
2. Ensure substance misuse and mental health groups, and Health and Wellbeing programmes, are facilitated/co-facilitated to the standard required by the Forward and in accordance with any relevant needs.
3. Carry out substance misuse and low level mental interventions relevant to individual client needs, including 1:1 sessions and group work/programme facilitation.
What are we looking for?
1. Knowledge of the issues facing substance misusers and low-level mental health issues.
2. Demonstrable experience of carrying out risk assessments, comprehensive assessments, and the design and implementation of SMART care plans.
3. Significant experience of carrying out psychosocial interventions including using motivational interviewing techniques in both 1:1 and therapeutic group settings.
4. A thorough knowledge of and commitment to the 12-step process of recovery from addiction.
5. Experience of providing counselling/CBT services to prisoners, or those with Health and Wellbeing, or drug and/or alcohol problems.
6. Have lived in the UK consecutively for the last 3 years.
