Job Title: Adults Learning Disabilities Social Worker
Location: Hammersmith and Fulham
Rate: Up to £36 per hour
Liquid Personnel is currently recruiting for an experienced Adults Learning Disabilities Social Worker in Hammersmith and Fulham. The pay rate is up to £36 per hour.
What will your responsibilities be?
The team is looking to appoint an individual who is enthusiastic to engage in adopting strengths-based, person-centered assessments. You will manage a varied caseload, which will include assessments and support planning for adults with learning disabilities, Care Act assessments, and coordinating with healthcare and support services. You will deliver exceptional social care to the adults we are supporting and their carers, promoting independence and well-being. You will engage in outcomes-focused and preventative conversations which build on the strengths of people and their local networks. You will work collaboratively with other agencies to ensure a coordinated approach to supporting adults with learning disabilities.

Qualifications and Experience:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK license
* Post-qualified experience
* Degree level or equivalent in Social Work
