Job summary The purpose of this role is to deliver care and support to patients, families, and carers at home for those identified as requiring care and support at end of life. Main duties of the job Demonstrate and maintain consistently high standards of individualised personal care and support under guidance of the Rennie Grove Peace Hospice at Home management team. Work alongside the patients' families and / or health care professionals, maintaining an approach that incorporates physical, psychological, social, and spiritual aspects of care in accordance with the Rennie Grove Peace philosophy. Observe and report changes in the patient's symptoms, conditions, and circumstances to the registered health professional, and notify other healthcare professionals as necessary. Work in accordance with the patient's agreed care plan. Support the patient with any physical / personal care, if the situation requires, maintaining dignity whilst doing. This may include a simple dressing or carrying out last offices if required. Responsible for identifying and acting on any risk of safeguarding to patients and their families in accordance with Rennie Grove Peace policies. Keep accurate and legible records in accordance with legislative requirements and Rennie Grove Peace practice and procedure. Prompt medication if dispensed in a pharmacy blister pack.
