We have an exciting opportunity to work as part of a new initiative to improve patient flow in our acute hospitals.
We provide a specialist service to sub acute, recovery and rehabilitation community beds aligned to our two district hospitals.
Our service aims to work as part of an MDT to provide support to patients who have been discharged from the acute wards to a community bed for continued rehab or support to enable them to return to the community.
This role will support those patients who require continued management of both communication and swallowing difficulties.
The beds will provide support for patients discharged from the stroke, frailty and orthopaedics wards.

Main duties of the job
The successful candidate will work as part of a specialist Community MDT supporting patients in two community wards, one in each acute hospital. At Telford there will be 20 stroke and frailty beds and at Shrewsbury, 32 frailty and orthopaedic beds.
You will have experience of working with dysphagia and acquired communication disorders within an MDT setting. The role will support patients with rehab and to aid patients transitioning from an acute ward back to the community.

Person Specification
Qualifications
* Degree or equivalent in Speech and Language Therapy
* Registered with Health and Care Professions Council (HCPC)
* Evidence of specialist clinical training in relevant areas
* Evidence of broad range of pre and post qualification experience
* Evidence of experience in relevant area of clinical practice - Dementia, neuro and dysphagia
* Evidence of teaching/ student experience
* Experience in audit and research
Knowledge
* Evidence of CPD (portfolio) detailing clinical experience relevant to the post.
* Knowledge and understanding of Clinical Governance
* Knowledge of current Evidence Based Practice relevant to post.
* Evidence of Specialist level of clinical reasoning
Skills
* Excellent written and verbal communication skills.
* Evidence of leadership ability
* Organisation, planning, prioritisation and decision making skills
* Clinical supervision, teaching, mentorship and appraisal skills

Salary: £37,338 to £44,962 per annum/pro rata
