Are you a qualified Social Worker passionate about working with people to promote independence and dedicated to making a difference? We currently have vacancies and are looking to further strengthen our Integrated Locality Team.
Early intervention is key to delaying or preventing the development of needs through community services and local information/advice. Our Social Workers facilitate and support needs assessments using a strengths-based approach, considering skills, abilities, and support networks. The resulting development of independence plans taking a person-centered approach, involves working with various agencies and professionals to meet service user/carer needs and promote joint working practices.
As such you will need to develop and maintain effective working relationships with colleagues and partner agencies, promoting multi-disciplinary working partnerships and will have proven ability to work as part of a team that puts the strengths of individuals, families and community at the heart of care and wellbeing.
As a qualified, SWE registered, Social Worker the key aspects of the role include:
undertaking assessments of individual and carers needs and risks to encourage independence, provide support and prevent harm.
working in partnership with service users and carers using a person-centred approach, with the aim of maximising independence, wellbeing and choice.
applying social work principles and values to guide professional practice and respect diversity.
