Bromley Healthcare are seeking qualified Health Visitors who are keen to develop their leadership skills and sphere of influence and are keen to shape the future of Public Health Nursing in Bromley, Bexley and Greenwich.

Main duties of the job
As a health visitor working in Greenwich you will provide quality face to face contacts for families for all key contacts, work in a diverse borough following the Healthy Child Programme.
* Identify health needs through an ecological assessment and in partnership with families, agree personalised plans to address the needs.
* Delegate aspects of care to the Community Nursery Nurses and Staff Nurses within the skill mix team.
* Engage and participate in service improvement.
* Continuously learn to develop your own skills and those of others.
* In partnership with clients and other partners, assess healthcare needs to develop and evaluate programmes to address the health needs of the population.
* Work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities.

Compensation is contingent upon NHS experience and current banding/pay point.

Detailed job description and main responsibilities
To deliver the Healthy Child Programme by:
* Carrying out ongoing health needs assessments for all children and families within the community.
* Overseeing the delivery of Universal service key touchpoints by skill mix team members.
* Delivering Additional Support Offer, Intensive support and Intensive Support Offer for Young Parents packages of care to families.
* Using Outcome measures to evaluate the effectiveness of care delivered.
* Participating and leading on the Early Help process for children.

* Diploma/Degree
* Registered General Nurse / Midwife / Mental Health Nurse/ Children’s Nurse/ Learning Disabilities Nurse
* Specialist Public Health Community Nurse / Health Visiting
* Qualified Nurse Prescriber
Professional Registration
* NMC registration as Nurse or midwife and Specialist Community Public Health Nurse
Skills
* Ability to work as part of a team
* Ability to work autonomously
* Ability to assess needs and to develop care plans
