Proposed working pattern to be discussed in interview -
Working Monday, Tuesday, Wednesday, Thursday, Friday working 0700am to 1000am then 1500pm to 2000pm (split shift)
You will also work every other Saturday & Sunday working 0700am to 1400pm.
We are seeking a dedicated and organised Care Coordinator to join our team in Cumbria. The Care Coordinator will be responsible for managing and coordinating care services during week days and weekends, ensuring that our clients receive continuous and reliable support.
Key Responsibilities:
- Coordinate and manage care services
- Respond to emergency calls and provide immediate assistance as needed.
- Schedule and allocate care staff to meet client needs.
- Maintain accurate records of all care activities and communications.
- Liaise with clients, families, and healthcare professionals to ensure seamless care delivery.
- Provide support and guidance to care staff during out-of-hours shifts.
Requirements:
- Previous experience in a care coordination or similar role.
- Excellent organisational and communication skills.
- Ability to work independently and make decisions under pressure.
- Compassionate and empathetic approach to client care.
- Proficiency in using care management software and other relevant tools.
Availability to work some weekends.
