South Tees Hospitals NHS Trust Community Collaborative and ICB Networks are working together to improve community rehabilitation services ensuring that the local population across Hambleton and Richmondshire to receive therapy close to home. As a result, there has been changes to service provision recently meaning that services can be delivered by the right therapist in the right place at the right time. South Tees Hospitals NHS Foundation Trust is one of the largest teaching hospital trusts in the country, close to the North Yorkshire Moors. It comprises two acute hospitals, James Cook University Hospital and Friarage Hospital Northallerton and several community hospitals and community services. We are recruiting to a permanent band 4 Therapy Assistant Practitioner post within Hambleton and Richmondshire Home from Hospital and UCR community therapy team which consists of Physiotherapists, Occupational Therapists, Therapy Assistant Practitioners. The service is provided over 7 days per week and the post holder will work between the hours of 8.00am-20.30pm. As services are delivered across the Hambleton and Richmondshire Locality there is an expectation that you will have an ability to work flexibly and travel independently within the community. Successful candidate must therefore have access to a vehicle for work purposes to meet the requirements of the role. Working in the community, you will provide rehabilitation in patients’ own homes and group settings, working closely with statutory and voluntary services to promote and maintain independence. There are supervision structures and competencies in place to ensure that you continue to deliver the best possible service to the patients in our care.
