Service Coordinator - Assisted Discharge Location: Gravesend, Kent Salary: £22,781 per annum pro-rata Hours: 21 per week Contract Type: Fixed Term Contract Until End of March 2025 Driving: Full UK Manual Driving Licence required and access to your own vehicle. Business mileage paid We are looking for a passionate and enthusiastic person with great people skills to join our Health and Local Crisis Response service as a Service Coordinator (Health & Care). If you have a caring, patient, and helpful nature and like communicating with people, then this role might interest you, as you will be taking referrals from hospital wards and managing the logistics of our service support team, enabling them in their role to help discharge patients’ home from hospital and providing a resettlement service for those who are less mobile. The role will be based in our Gravesend British Red Cross (BRC) Office but will also involve attending meetings and networking in the Darent Valley Hospital & other associated NHS locations in the catchment area to help promote and market the service. This role is for 3 days a week, during the Monday to Friday. (The exact days of the week can be negotiated.) To manage the BRC Assisted Discharge Service (ADS) ensuring it is of high quality and meets service standards. To take referral calls from hospitals and manage the logistics of the service delivery. To promote the service, including attending hospital staff meetings and networking events. Carry out daily/weekly in reach within the hospitals. Work closely with other voluntary organisations to ensure appropriate support for service users, carers and the people they care for. Provide cover where support workers are not available, or service demand is increased (hospital pressures). Ensure high quality and safe services (meeting requirements of BRC’s quality standards framework, and health and safety, hygiene/infection control, and safeguarding procedures). Ensure that service performance is monitored, maintained, and delivered consistently with national and local guidelines, using the Quality Standards Framework What do you need to be a successful Service Coordinator?
