The Isle of Wight NHS Trust (IWT) welcomes you to join us on our journey towards this goal. IWT are thrilled to offer a fantastic opportunity to join our Pharmacy Team. We are recruiting Foundation Pharmacists (Band 6-7 progression posts) and Specialist Rotational Pharmacists (Band 7 posts).
The Pharmacy Team based at St Marys Hospital are energetic and forward looking, with a strong culture of patient safety and service quality. We believe in technology supporting excellent clinical practice and releasing time to care, with Electronic Prescribing and Medicines Administration (EPMA) having been embedded in practice for over 10 years.
You will get the chance to work in many clinical areas including our Emergency Department, Acute Assessment Unit, General Medicine, Stroke, Care of Older People, General Surgery, Orthopaedics, Oncology, Haematology and Hospice care.

Job responsibilities
Foundation Pharmacists Key Responsibilities:
* Complete Trust funded programme (2 Years). We will support you to develop your skills to match the RPS Foundation Pharmacist Framework.
* Provide support and underpinning of the clinical work of senior clinical pharmacists.
* Support to Urgent and Emergency Services, Medical, Surgical and Orthopaedic wards, Stroke and Coronary Care Services, Hospice, Medicines Advice services, and dispensary/discharge activities on a rotational basis.
* You will undertake Medicines Reconciliation and core clinical pharmacy duties.
* You will ensure systems for facilitating discharge medicines to meet patients and general practitioners needs.
* Be actively involved in the training and development of pharmacy team members.
* Support in education and health promotion with patients and the public.
* Provide a core weekday service with some weekend and on-call duties.
* On-call pharmacist duties are undertaken from home.
Specialist Rotational Pharmacist Key Responsibilities:
* You will work closely with the multidisciplinary team and attend Consultant-led ward rounds to inform and influence prescribing decisions.
* Participation in rotations providing support to Oncology/Haematology, frailty and older person care, Urgent and Emergency Services.
* Prescription monitoring and provision of pharmaceutical care plans ensuring the active involvement of patients and carers.
* Ensures the continuity of pharmaceutical care by provision of drug history, prescription monitoring, clinical monitoring of drugs and discharge planning.
* You will be supported to complete your foundation training as a pharmacist and develop your specialist skills.
* Be provided training to support your leadership and management skills.
* Be actively involved in the training and development of pharmacy team members.
* Actively support medicines safety including supporting the investigation of medicines safety incidents.
* Provide line management support to pharmacy team members.
* Support in education and health promotion with patients and the public.
* Represent pharmacy in Trust meetings.
* Provide a core weekday service with some weekend and on-call duties.

Qualifications
Essential
* Masters degree in Pharmacy.
* Membership of General Pharmaceutical Council.
Experience

* Ability to Prioritise.
* Show a flexible approach to changing circumstances.
* Have an innovative approach to problem solving.
Desirable
* Post graduate certificate in pharmacy practise.
